Fill out the HANDPIECE REPAIR FORM completely and place it along with your sterilized handpieces in the courtesy repair box. If you don't have a courtesy repair box you can request one via the Request Courtesy Repair Box link or call your Account Manager at DHPI (800-209-0927) and we will send you one no charge. Be sure to keep a copy of the HANDPIECE REPAIR FORM for your records.

Please follow the directions on the form carefully, fill it out as complete as possible to help avoid unnecessary delays.
